However, these growths can sometimes indicate a more serious condition: salivary gland cancer. While initially painless and often overlooked, these tumors require careful diagnosis due to their varied locations and potential impact on facial function.
What is Salivary Gland Cancer?
Salivary gland cancer originates from the tissues that produce saliva. These cancers are relatively rare, accounting for a small percentage of all head and neck cancers. The salivary glands are divided into major and minor types. Major salivary glands include the parotid glands (located in front of and below the ears), submandibular glands (under the jaw), and sublingual glands (under the tongue). Minor salivary glands are found throughout the lining of the mouth and throat.
The location of the tumor plays a crucial role in its presentation and potential complications. Tumors in the parotid glands, for instance, are the most common type of salivary gland tumor and can present as a visible lump. Cancers arising from minor salivary glands, often located inside the mouth, may initially cause discomfort, bleeding, or difficulty swallowing.
Types of Salivary Gland Cancers
Salivary gland cancers are classified based on the type of cell from which they originate. Common types include:
- Adenoid Cystic Carcinoma: Known for its slow growth but potential for nerve invasion and recurrence.
- Mucoepidermoid Carcinoma: The most common type, ranging from low-grade (slow-growing) to high-grade (more aggressive).

- Facial numbness or weakness: Particularly if the tumor affects the facial nerve, which runs through the parotid gland. This can lead to difficulty closing an eye, drooping of one side of the face, or altered facial expressions.
- Pain: While often painless initially, tumors can cause pain as they grow or invade surrounding tissues.
- Difficulty swallowing or opening the mouth: Especially with tumors located in the deeper parts of the salivary glands or those affecting the throat.
- Changes in saliva production: Such as dry mouth.
- Numbness or tingling: In the area of the tumor.
- Swollen lymph nodes: In the neck, which may indicate the cancer has spread.
The proximity of the parotid gland to the facial nerve is a critical factor. Tumors in this area can compress or invade the nerve, leading to facial paralysis or weakness. This nerve involvement is a significant consideration during diagnosis and surgical treatment.
Diagnosis and Staging
Accurate diagnosis is paramount for effective treatment and prognosis. The diagnostic process typically involves several steps:
- Physical Examination: A thorough examination of the head, neck, and mouth to identify lumps, swelling, or other abnormalities.
- Imaging Tests:
- CT Scan and MRI: These imaging techniques help determine the tumor’s size, location, and extent, including whether it has spread to nearby nerves, blood vessels, or lymph nodes.
- PET-CT Scan: May be used to assess if the cancer has spread to distant parts of the body.
- Biopsy: This is the definitive diagnostic step. A sample of the tumor tissue is removed and examined under a microscope by a pathologist to confirm cancer and determine its specific type and grade. Various biopsy methods may be used, including fine-needle aspiration (FNA) or a surgical biopsy.
Staging of salivary gland cancer involves assessing the tumor’s size, its involvement of nearby structures (like nerves or lymph nodes), and whether it has metastasized (spread) to distant organs. This information guides treatment decisions and helps predict the patient’s outlook.
Treatment Options
Treatment for salivary gland cancer depends heavily on the type, stage, grade, and location of the tumor, as well as the patient’s overall health.
- Surgery: This is the primary treatment for most resectable salivary gland cancers. The goal is to remove the tumor completely with clear margins (a border of healthy tissue around the tumor). For tumors involving the parotid gland, surgery often requires careful dissection to preserve the facial nerve. In cases of extensive nerve involvement, nerve reconstruction may be necessary. Neck dissection may also be performed if there is a risk of spread to the lymph nodes.
- Radiation Therapy: Often used after surgery, especially for high-grade tumors, those with positive surgical margins, or when the cancer has spread to lymph nodes or surrounding tissues. It can also be a primary treatment option for tumors that cannot be completely removed surgically or for patients who are not candidates for surgery.
- Chemotherapy: While less effective as a standalone treatment for many salivary gland cancers, chemotherapy may be used in combination with radiation therapy or for advanced or recurrent cancers, particularly those that have spread to distant sites. Its use is often tailored to the specific type and characteristics of the cancer.
Prognosis and Outlook
The prognosis for salivary gland cancer varies widely. Factors influencing the outcome include the tumor’s type, grade, stage at diagnosis, the success of surgical removal, and the patient’s response to treatment. Low-grade, early-stage cancers generally have a good prognosis, while high-grade or advanced cancers may have a more challenging outlook. Regular follow-up care is essential to monitor for recurrence and manage any long-term effects of treatment.
Dr. Kang Min-seok, a professor of Otorhinolaryngology at Soonchunhyang University Seoul Hospital, emphasizes the importance of seeking medical attention for persistent lumps. “If a lump that grows slowly and painlessly appears near the ear or under the jaw and persists for more than one to two months, especially if accompanied by symptoms like facial paralysis or swollen lymph nodes in the neck, it is crucial to get it examined,” he stated. He further advised, “Although definitive diagnosis before surgery can be challenging due to the diverse nature of salivary gland tumors, a comprehensive assessment through pre-operative tissue examination and imaging studies is vital. If malignancy is suspected, prompt surgical intervention is the most critical factor for both cancer treatment and functional preservation.”
Early detection and a multidisciplinary approach involving surgeons, oncologists, and radiation oncologists are key to achieving the best possible outcomes for individuals diagnosed with salivary gland cancer.
